Foreign Credit Card Authorization Form

This form is necessary in the following circumstances:

(1) You are using a foreign credit card for which we can't verify the billing address or zip code or
(2) Your address is different from the billing address of the card or
(3) We encounter problems such as disconnected telephone numbers in our attempt to verify your order or
(4) Your advertising total is greater than $500.00 USD; we must have the card holders signature.

Rules:
(1) Please fill out this form after placing your order.
(2) You only have to fill out this form once unless your billing address changes or you pay with a different card.
(3) We cannot guarantee that your advertisement will be placed immediately after receiving this form by fax. Further verification, although rare, may be necessary under some situations.
